How to Arrange for Services at Home

 Hospitalization is not a prerequisite for home health care services. If you are hospitalized or admitted to a rehabilitation center, inform the discharge planner that you would like the services of the ELITE Home Health Agency.

 Discuss your needs with your physician who will contact the ELITE Home Health Agency. You and your family may contact the agency directly.  A Quality Care Specialist will consult with you to set up, in collaboration with your healthcare professionals, an individualized plan tailored to your personal needs.
